Abstract
The kidneys are the primary organs for maintaining homeostasis in the human body, and any alteration in their function will impact their performance. This study aims to highlight the importance of identifying dose adjustments for standardized medications in municipalities in the Baixada Santista region for patients with renal failure. REMUME data from the nine municipalities in the Baixada Santista region and the MICROMEDEX® database were used to assess the need for dose adjustments. An average of 22.31% of standardized medications in municipalities in the Baixada Santista region require dose adjustments for administration to patients with renal failure. Implementing automated institutional protocols or checklists for dose adjustments based on renal function is essential
