Abstract

Intravenous medication incompatibilities are dangerous and far from uncommon. The objective of this study was to analyze the Y-shaped compatibility between standardized intravenous medications in a public hospital in Santos, São Paulo. The theoretical framework used was PUBMED®, a survey of standardized intravenous medications at the hospital, and data available on its internal network (intranet). Micromedex® was used to assess Y-shaped compatibility by cross-referencing information in an Excel® spreadsheet to identify incompatibilities. The final analysis identified 2.655 documented incompatibilities, a significant number of undesirable interactions, highlighting the essential role of pharmacists in ensuring safe medication administration in the hospital
