Long-term hospitalization of patients with neurological sequelae after stroke in Rio Grande do Sul A public health problem

Abstract
Stroke is the most frequent cause of death in adults in
Brazil. It is characterized by the sudden onset of any neurological defects. The sequelae presented by the survivors characterize an important public health problem.This study aims to investigate how long-term admission lawsuits for these individuals are served by the Rio Grande do Sul Judiciary, in comparison with the recommendations of the literature. Methods To do so, we analyzed decisions rendered by the Court of Justice of the State of Rio Grande do Sul, in civil proceedings, from July 1, 2013 to June 30, 2018, with requests for long-term admission. The judgments were obtained on its jurisprudence site, in response to chosen descriptors. The results were compared with the literature. During this five-year period, eleven judgments granted long-term admissions in specialized places, or in home care or even in a general hospital. There was no allowance of hospice admissions. A judgment acknowledged the lack of municipal institution proper to this type of admission. The most frequent funding of afforded admissions was public. WHO and CREMESP recommend long term admission in hospices. Such care should be integrated into a network that includes other institutions, to attend specific situations. In only 14.3% of the cases, this occurred in the sample considered. In Rio Grande do Sul, unlike the literature, there are few institutions able for this service. Hospitalization in general hospitals should be avoided. Effective public policies should be implemented to provide long-term care for stroke sufferers.
